It includes personal information such as name, date of birth, and vaccination records (vaccine type, date of administration, lot number, etc.). Additionally, for international use, it also includes nationality and passport number.

The vaccination certificate contains a QR code that encompasses information such as name, date of birth, and vaccination records. Scanning this code allows for easy verification of information.

2. 4-digit PIN for My Number Card surface input assistance
3. Passport (for those obtaining international use)
